Clear Bloodmoon Island, learn the tree’s name, talk to it, go to Arx and confront the Doctor. Or just confront him w/o any act 2 stuff. Saheila isn’t required at all for Sebille’s story. Act 3 will give all the context you need even w/o her there, and she plays no role in act 4 either way. Steal items and then kill them.
